To declare a Physics major or minor, fill out the Declaration of Major/Minor Form available in OPUS (located under the Planning and Enrollment section). After submitting the form through OPUS, you'll get an email instructing you to reach out to the Physics Undergraduate Academic Degree Coordinator, Susan Cook. You'll also receive an automated confirmation email from the physics department acknowledging your DOM/M submission.

Your submitted form will be forwarded to the Physics department for evaluation. Upon approval of your DOM/M, Susan Cook will send you a final welcome email representing both the Physics Department and the Director of Undergraduate Studies. This email will include your assigned physics advisor's details along with additional exciting information about the physics program.
